Evaluation Day

Please bring the following information:

  •     Writing samples
  •     Any samples of school work that include reading, spelling, math
  •     Copies of all formal testing and any recent IEP or 504 documentation
  •     Any information that you believe would be beneficial


Testing Time:

Plan to test over a two day period of time. Typically, the first morning (Monday) will begin at 8:15 AM and will break for lunch at noon. The afternoon testing will begin at 1:00 PM and end around 3:30 PM. The second day (Tuesday) will begin at 8:15 and end around 12:30 PM. An individual's age and attention span will determine the exact times. Please plan on two full days.


Conference and Testing Results:

The tests will be interpreted and a formal report will be provided as quickly as possible (average time is within 15 work days), with a maximum of 30 work days after testing. A conference will be held as soon as the report is completed. Recommendations will be given, and questions will be answered at this meeting.


Fee Structure for Individuals Only:

Total fee of $1100.00 includes: Individualized testing, report, and final conference

Please note: The $100.00 is nonrefundable and will be collected when appointments are made.

A payment in the amount of $500.00 will be required the first day of testing and is not refundable. The balance of the fee will be due ($500.00) and payable by the client upon receipt of the formal report on the day of the conference. There is no charge for the conference where the formal report is discussed. A fee of $20.00 will be added each time a credit card is used or $40.00 if the full payment of 1000.00 is paid at one time on a credit card. The initial $100.00 to reserve a date will not be charged the extra $20.00.
